The low back is the one to watch

Low back accounted for 16.7% of injuries in the collegiate study, the largest single site. A third of the lumbar injuries were spondylolysis, a stress injury to the bony arch of a vertebra, and three quarters of those ended the athlete's season. Adolescent back pain that keeps coming back after extension work is worth having imaged rather than trained through. That is a physician's call, not a coach's.

Most of it is muscular and most of it is at takeoff

Strains made up 39.2% of injuries and overuse another 25.5%. Thirty percent of all injuries happened during the plant and takeoff, which is the moment the vault asks for the most from the hamstrings and the shoulder in the same instant. Hamstrings and lower leg tied for second place at 13.9% each. Lower extremities carried 60% of the total load.

Prior injury is the strongest signal you get

Vaulters who came into the season with a history of multiple injuries had 2.7 times the odds of getting hurt again. That is the most actionable finding in the paper. If you spent last spring in and out of the training room, the honest read is that your progression, not your bad luck, needs looking at before you add grip height.

What is the most common pole vault injury?
In the collegiate study, low back injuries were the most common single site at 16.7% of events, with hamstrings and lower leg next at 13.9% each. By type, muscular strains led at 39.2%. Sixty percent of all injuries were in the lower extremities, which fits an event that is mostly sprinting.
How dangerous is pole vault really?
Catastrophic injuries do happen and they are rare enough that the research counts them one at a time. A prospective study covering 2003 to 2011 in the United States recorded 19 catastrophic injuries across nine years, an average of 2.1 per year in the whole country. Fatalities over that period averaged 0.22 per year, down from about 1.0 per year in the era before the 2003 landing pad rule changes.
What causes the most serious pole vault injuries?
Landing in or around the box. In the 2003 to 2011 study, 74% of catastrophic injuries involved landing in or near the vault box, 21% involved missing the pad entirely, and 5% involved a broken pole. The earlier 1982 to 1998 review found bouncing off the pad onto hard ground was then the leading mechanism at 53%, which is what the enlarged pad rules were written to address.
Is it true 1 in 100,000 vaults ends badly?
That figure gets quoted a lot and neither of the major catastrophic-injury studies actually publishes it. Both report counts of injuries per year across the country, not a probability per attempt, because nobody counts total vaults taken. Treat any per-vault odds you see as an estimate somebody built, not as a measured rate.
Should I train through pole vault back pain?
That is a question for a physician or an athletic trainer, not for a website. The reason it matters in this event specifically is that a meaningful share of vaulters' lumbar injuries in the collegiate study turned out to be spondylolysis, which is a bone stress injury and mostly ended seasons. Persistent back pain in a young vaulter is worth getting looked at.
